Family Healthware™, developed and tested by the Center for Disease Control and Prevention (CDC), provides tools to predict common chronic health conditions, such as diabetes, stroke and cancer, and take steps to prevent them.

Family Healthware's free service employs a quick, easy-to-use questionnaire including health behaviors, medical screenings, and the medical history of first and second-degree relatives. Members may invite relatives to create a comprehensive family health tree and health history record for their family. 

The CDC evaluated Family Healthware at the University of Michigan School of Medicine, the Evanston Northwestern Healthcare Research Institute, and the Case Western Reserve University School of Medicine.
